Police Records Request

The form must be filled out in it's entirety and can be emailed to: records@springlakeparkmn.gov  or dropped off at our officer during business hours.  There is a charge of .25 cents per page. Reports may be emailed on a case by case basis. If you live in the metro area and are requesting a police report, you will be asked to come in to pick it up. A data request form will need to be filled out prior to the report being released. If you are not the subject of the data, we will need a signed release from signed by the subject of the data.
