Police Records Request

Fill out the form and bring it in to the police department. If you are unable to do so, you may email to: Records@slpmn.org. There is a charge of .25 cents per page. Reports may be emailed on a case by case basis. If you live in the metro area and are requesting a police report, you will be asked to come in to pick it up. A data request form will need to be filled out prior to the report being released. If you are not the subject of the data, we will need a signed release from signed by the subject of the dat.
